Student Athlete Forms

The following forms are required prior to participation with Olympic College Athletics.

 

In ordering to complete these forms, please gather the following information:

  • CTC Link Number
  • Medical Insurance Information
  • Medical History

For all incoming Ranger student-athletes, please complete the following: 

  1. FERPA Release (pdf) - please download, fill out and send to your Head Coach. 
  2. NWAC Recruiting Disclaimer (pdf) - please download, fill out and send to your Head Coach. 
  3. Information & Photo Release Form (pdf) - give completed form to your coach
  4. Olympic College Medical History Form  (pdf)- give completed form to your coach
  5. Physical Form (pdf)- required for all first-year student-athletes. You may also use the form your doctor provides.
  6. Risk & Release Form (pdf) - give completed form to your coach:
    1. Baseball
    2. Basketball
    3. Cross Country/Track & Field
    4. Golf
    5. Softball
    6. Volleyball
